Credential Check
All professional licenses and credentials are verified (with copies).
Proof of Insurance
We require our anesthesia professionals to carry their own professional liability insurance ($1 million / $3 million malpractice coverage, or state-specific requirements). We (Coastal) also carry our own professional liability insurance.
Criminal Background Check
We verify and validate.
References
We require at least three completed professional reference forms. These are checked and verified.
Employment History
We verify all past employment.
NPDB Query
A National Practitioner Data Bank (NPDB) query is completed on each contracted professional to check for things like license revocation, reprimands, censure, suspensions, etc.
Application/Contract
We require all contractors to sign an application and a contract agreement, which we keep on file.
Identification
We require a valid picture ID.
CV
Updated / current Curriculum Vitae.
Competency Overview
An initial anesthesia competency overview is completed and verified.
Evaluation
An annual facility competency evaluation is completed.
NPI Number
Eg, each contractor is required to have a National Provider Identifier (NPI) number.
Joint Commission Competency
We provide each facility with Joint Commission-credentialed files for each professional.
